SIGNUP GUARANTEE
Submission of this form does not guarantee a spot in the class. If multiple submissions come in at once and the class is full, you will be placed on the “Wait List”. Your spot is only confirmed once you receive an email from “The Charnel Box”. Please wait 24-72 hours for spot confirmation.  Spots will be forfeited if your payment is not received  7 days before class. (Click here for more details)

AGE REQUIREMENT
Students ages 12+ are welcome. A consent form from the guardian is required along with the standard registration form if under 18 years old. Complete this form if under 18 (Click here for more “Under 18 Form”)

LOCATION
The workshop will be taught at our home workshop in Colorado Springs. We are located 5 miles south of downtown Colorado Springs, near the Cheyenne Mountain Library in Ivywild. Exact address will be provided 72 hours before class.

STEP 2: Payment

Once you complete the form below, you will be asked to pay. If you cannot pay in full at the time of signup, a minimum $100 deposit is required for all workshops at the time of registration with the remaining balance being due 1 week (7 days) prior to the workshop. If your full payment amount is not received 1 week prior to the workshop, your spot will be forfeited. Due to the nature of our work, all deposits and payments are final and not refundable- for any reason.

Informed Consent + Acknowledgement

I hereby assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold Sal/The Charnel Box harmless and all its respective affiliates and representatives from any and all liability for injuries to said participant arising out of participating in the class. In case of injury to participant, I hereby waive all claims against Sal/The Charnel Box, affiliates, participants, and, if applicable, owners and lessors of premises used to conduct the event.

Photo Release Form

Form submitter gives permission to use photographs taken during the class for an legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, and web content.

Payment + Refunds

$100 deposit is required for all workshops at the time of registration with the remaining balance being due 1 week (7 days) prior to the workshop.
